The Collective

Optimizing health through personalized medicine and nutrition. After the initial consultative appointment with Dr. Wiley and Sophie Purke, you will have 2 subsequent follow-up appointments with Dr. Wiley and 5 follow-up appointments with Sophie Purke for nutrition over a 3-month minimum span. The package includes limited access to the practitioners via text message throughout the month.

Integrative and physical medicine and nutrition

3 month package which includes physical therapy with Dr. Pollan once a week for the 1st month, 2 times a week for the remainder of the package, 2 appointments a month with Sophie Purke, and 1 appointment per month with Dr. Wiley.

Initial Consultation: A thorough 1-hour appointment dedicated to understanding your unique needs and concerns.

Follow-up Sessions: Two 20-minute appointments designed to review laboratory results and track progress together in a supportive environment.

A La Carte

$300 – 60 Minute New Patient Appointment (Required for all patients who have not seen Dr. Wiley in the last 3 years)

$250 – 40 Minute Established Annual Well Woman Appointment

$125 – 20 Minute Appointment

Follow up for an established patient. Includes reading and interpreting functional labs.

$125 – 20 Minute Appointment – A session that caters to infection management and acute pain assessment only.

Procedures

$275 – An appointment for IUD insertion/retrieval, colposcopy, or polypectomy.

$350 – HRT Pellet Insertion/Retrieval (Must have Pellet Consult before Pellet Insertion, please schedule in order, otherwise pellet insertion will be cancelled)

$400 – Nexplanon Insertion/Retrieval
