All of our clinicians are committed to providing individualized, thorough care. For us, this means we function outside of the health insurance system. Appointment pricing and payments are handled directly with each provider. HSA/FSA funds are accepted.
From complex health concerns to proactive wellness, our collective brings together a range of expertise to help you find the care that’s right for you.
I’m Brooke Jordan, a certified Functional Medicine Practitioner through the Institute for Functional Medicine and, probably most importantly, mom to two wonderful kids, Nolan and Evelyn.
With a twenty-year background in obstetrics and gynecology, I have shifted from delivering babies to partnering with women to address fertility, hormones, menopause and chronic disease through a functional lens.
One of my favorite compliments I’ve gotten from a client recently was “therapy with you doesn’t feel cheesy. It’s real.” And I hope you will find that to be true as well - that together we can be authentic and real and work on the hard stuff that might feel too big or messy to handle alone.

I'm Allison-I work alongside Dr. Wiley, helping with patient communication and management to make sure everyone gets the care they need. With 15 years of nursing experience and a BSN from the University of Oklahoma, I've built a solid foundation for supporting patients along the way. Hi, I'm Sarah! As a Care Coordinator at Fort Worth Health Collective, I love helping patients find the right provider for their unique needs. I earned my BSN from Baylor University, so I bring a solid nursing background to how I think about patient care, even though I'm not currently practicing as an RN. I’m Brooke Jordan, a certified Functional Medicine Practitioner through the Institute forFunctional Medicine and, probably most importantly, mom to two wonderful kids, Nolanand Evelyn...
My journey to functional medicine began during my years practicing as a physician in large hospital systems. I often felt frustrated by the limitations of conventional care: not enough time with patients and too few lasting improvements in their health. I wanted a better way. Discovering functional medicine gave me that path.Functional medicine addresses the whole person, the connections between body systems, the underlying causes of illness, and the fundamental role lifestyle plays in healing. This approach deeply resonated with me, and it has since become my passion and calling.
Today, I work with a wide range patients, male and female, from adolescence through later adulthood, helping them not only regain health but also sustain it for the long term.I earned my medical degree from the Texas College of Osteopathic Medicine at theUniversity of North Texas Health Science Center and completed my Family Medicine residency at John Peter Smith Hospital in Fort Worth.
I became board certified in FamilyMedicine in 2016 and later pursued advanced training in functional medicine, earning certification from the Institute for Functional Medicine in 2025.
Outside of my practice, you’ll find me enjoying life with family and friends, reading, and spending time outdoors at our family ranch. These moments recharge me and inspire the balance I strive to help my patients achieve.
I am driven by a desire to guide my patients toward optimal health and wellbeing through personalized, woman-focused care. With a twenty-year background in obstetrics and gynecology, I have shifted from delivering babies to partnering with women to address fertility, hormones, menopause and chronic disease through a functional lens. These objectives are challenging to attain within our traditional, problem-based insurance model of medicine.
I am driven by a passion for preventive care and optimizing for a vibrant, healthy life. I combine cutting edge research with compassionate guidance to foster health and vitality at every stage. I am committed to reshaping healthcare, ensuring that every woman receives comprehensive and tailored support for her unique health journey.
I received my undergraduate degree in psychology from UT San Antonio. I went on to attend medical school at UNT Health Science Center in Fort Worth. I completed OBGYN residency at John Peter Smith Hospital where I served as chief resident. I joined the faculty of the Baylor Scott and White/TCU Obstetrics and Gynecology Residency in 2021 where I am fortunate to work with physicians in training.
On the path to certification through the Institute for Functional Medicine, I have completed extensive course work in hormones, cardiometabolic health, Gl function and inflammation, immunity and environmental exposures. I am a North American Menopause society certified menopause practitioner.
When not working, I have an ever-evolving yoga practice and love hiking, reading and travel. I am a recovering marathoner but still love the occasional run on a crisp fall day. As a wine lover I am a Level 2 sommelier through the Wine and Spirit Education Trust. I was born and raised in San Antonio and am a devoted Spurs fan. In my free moments I spend time with my husband, children, and grandchildren who are the great joys of my life.
One of my favorite compliments I’ve gotten from a client recently was “therapy with you doesn’t feel cheesy. It’s real.” And I hope you will find that to be true as well - that together we can be authentic and real and work on the hard stuff that might feel too big or messy to handle alone. Finding the right counselor can be a daunting task, but I believe the best counselor for you is going to be the one with whom you have the best relationship. The one you click with. We know that successful outcomes from counseling come not as much from the training and background of the counselor, though those things matter too, but more so from the dynamics between counselor and client.
I am EMDR certified but use many modalities, custom fit to find the best approach for each client who walks through my door. I have a true passion for all things related to perinatal mental health, based on my own experience when having my children. Complex trauma is my specialty, and I have worked with many over the years who struggle with chronic mental health concerns, including depression and suicidal ideation. If you have questions about what the counseling process might look like or how I can help you, reach out and let’s chat.I consider my profession to be a privilege as I know how difficult it can be to be open and vulnerable about life’s hardest moments.
What an honor it has been to sit with so many clients in their lowest lows, and just as much to rejoice with them in the highs. I always have hope for my clients, and I will work to make sure you find your own but, in the meantime, I’m happy to let you borrow some of mine.
Outside of work, my favorite people call me “Mama” and I love spending time with my family, including my two very cute kids. I am active in my church and enjoy playing piano in the praise band. Music has always been my favorite form of self-expression and therapeutic release. I am a die-hard Green Bay Packers fan and am a proud Midwesterner by birth. My love languages are pizza and beach vacations.
Counseling is serious work, but it doesn’t always have to be serious and I love a good counseling meme.

Dr. Marie Woerner PT, DPT, WCS, CLT graduated from the Texas Tech Health Science Center with her Doctorate in Physical Therapy in 2008. After graduating she took the advice of many experienced mentors and practiced at a busy outpatient orthopedic clinic here in Fort Worth, TX. There she had the opportunity to treat a variety of diagnosis from low back pain to turf toe. In addition to these diagnoses, many of her patients also included postoperative shoulders, knees, spine, and ankles, allowing her to develop her orthopedic manual skills and individualized practice style.
After several years of treating orthopedic patients, Marie transitioned into Women’s and Men’s Health by completing an American Physical Therapy Association (APTA) approved residency. During her residency she had the opportunity to mentor with top clinicians in this specialty. Upon completion of her residency, Marie was offered a clinical faculty position with the University of North Texas (UNT) Health Science Center and began to develop the Pelvic Health Program for the University.
During her time at UNT she was able to grow a thriving practice, as well as obtain her Board Certification in Women’s Health, making her one of the only Board Certified Specialists recognized by the APTA in Tarrant County. She also developed a strong interest in Functional Dry Needling and obtained her certification through Kineticore, where she was able to learn from many outstanding clinicians. Marie has a passion for helping others, spreading the word about pelvic health, and strives to provide exceptional care to all her patients.
